We want to incorporate the "Incident - Non-Latent Variation Request form-Field Manager" into siteTRAX and have many of the fields auto populated.

This form needs to be available in the Incident programme of work. The Geotag fields need to be auto-populated based on the JPS filled out by the tech. This will automatically provide the time the tech spent onsite working on the job.

The form needs to capture where the geotag was captured (at the job address or --km's from the job)

The code the tech is claiming needs to be pulled from the "Incident e-form" along with the NDR information if applicable.

The form needs to calculate the amount of additional time the tech may be entitled to claim on top of the code he is claiming (each code has a portion of time allocated to it)

The form also needs to capture multiple visits if applicable. This will also be captured by the JPS and the Geotag function completed by the tech on each visit.

The tech will need to be able to attach photos to this form to support his claim.

The techmate results (before and after) will need to be captured and shown in this form

Based on the code claimed by the tech and the additional time he is claiming there will be a percentage of margin shown for the FM/RM to view. This should not be available for the tech/DP to view.

Based on the information revealed in this form the FM/RM can decide on whether the top-up is approved or not.
